Pearlcium was developed and marketed by Pharm East, Inc., a natural-products company founded in 1996 by Dr. Rulin Xiu. Xiu earned a doctorate in physics from the University of California, Berkeley, before establishing Pharm East and later developing manufacturing operations in China and Hawaii. During the 2000s, the company introduced Pearlcium International as the network marketing division for its pearl-based nutritional products. The business operated from Keaau on Hawaii's Big Island and held its first national distributor convention in Las Vegas in September 2009.

Pearlcium's primary product was a finely processed pearl powder sold as a source of calcium, amino acids, trace minerals, and naturally occurring pearl proteins. The company promoted a proprietary manufacturing method that did not use heat or acidic solutions, which it said preserved the pearl's crystalline structure and protein content. Pearlcium was marketed for general wellness, bone and dental health, skin appearance, relaxation, sleep, and healthy aging. Historical products associated with the company also included Sleep the Pounds Away AM and PM and Ibistrol. Pearlcium International sold through independent distributors using a multilevel marketing model.
